To Get Started
We ask that to start with us, you first fill out our intake form. It's a quick online form that will notify our staff of your medical need
If you are a new patient, please fill out our intake information here
Our intake consists of questions regarding your information, your insurance and your medical history. This is so we are able to continue with the next steps in your recovery.
Once you have completed your intake form, our staff will verify your insurance and request a perscription from your doctor. When we have both of those things back, we will call you to either schedule you for an apointment, or start ordering your garments.
